As part of UCCS’ preparations for returning to a more fully in-person campus experience this fall, all students, faculty and staff are required to respond to the vaccination attestation. The attestation is in line with the University of Colorado’s April 28 decision to require COVID-19 vaccinations for students, faculty, and staff.
The form is open to UCCS employees. Students received a separate communication with instructions to submit their attestation.
The attestation form requires individuals to attest to one of the following:
- I am fully vaccinated* against COVID-19.
- I am not yet fully vaccinated against COVID-19; however, I intend to receive a COVID-19 vaccine or the second dose in a two-dose series. If I do not receive the vaccine, I will update this attestation.
- I am not fully vaccinated against COVID-19, and I request an exemption from the COVID-19 vaccination requirement.
*According to the CDC, individuals are considered fully vaccinated two weeks after their second dose in a two-dose series (such as the Pfizer or Moderna vaccines) or two weeks after a single-dose vaccine (such as Johnson & Johnson’s Janssen vaccine).
Completed attestations will be maintained in a secure system adhering to UCCS’ privacy policies and compliance practices. Information concerning an employee’s completed attestation will not be shared and cannot be requested by supervisors. Data will only be shared in aggregate to determine our progress toward campus herd immunity.
