Student vaccine attestation reminder

Today, all UCCS students should have received an email from the Wellness Center requiring you to fill out the vaccination attestation form. As part of UCCS’ preparations for the fall semester, all students, faculty and staff will be required to respond to the attestation. The attestation is in line with the University of Colorado’s April 28 decision to require COVID-19 vaccinations for students, faculty, and staff. 

The attestation form requires individuals to attest to one of the following: 

  • I am fully vaccinated* against COVID-19. 
  • I am not yet fully vaccinated against COVID-19; however, I intend to receive a COVID-19 vaccine or the second dose in a two-dose series. If I do not receive the vaccine, I will update this attestation. 
  • I am not fully vaccinated against COVID-19, and I request an exemption from the COVID-19 vaccination requirement. 

* According to the CDC, individuals are considered fully vaccinated two weeks after their second dose in a two-dose series (such as the Pfizer or Moderna vaccines) or two weeks after a single-dose vaccine (such as Johnson & Johnson’s Janssen vaccine). 

Completed attestations will be maintained in a secure system adhering to UCCS’ privacy policies and compliance practices. Information concerning a student’s completed attestation will not be shared and cannot be requested by other students, faculty or staff. Data will only be shared in aggregate to determine our progress toward campus herd immunity and/or to contact students if they have been exposed to known positive cases, not for the purposes of limiting individual student access to courses, programs and services.
