The attestation is in line with the University of Colorado\u2019s April 28 decision to require COVID-19 vaccinations for students, faculty, and staff.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a rel=\"noreferrer noopener\" href=\"https:\/\/uccs.co1.qualtrics.com\/jfe\/form\/SV_6mcpgactaMLTeLQ\" target=\"_blank\">The form is open to UCCS employees<\/a>. Students received a separate communication with instructions to submit their attestation.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link\" href=\"https:\/\/uccs.co1.qualtrics.com\/jfe\/form\/SV_6mcpgactaMLTeLQ\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Fill Out<\/strong> <strong>Attestation Form<\/strong><\/a><\/div>\n<\/div>\n\n\n\n<p class=\"wp-block-paragraph\">The attestation form requires individuals to attest to one of the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>I am fully vaccinated* against COVID-19.<\/li><li>I am not yet fully vaccinated against COVID-19; however, I intend to receive a COVID-19 vaccine or the second dose in a two-dose series. If I do not receive the vaccine, I will update this attestation.<\/li><li>I am not fully vaccinated against COVID-19, and I request an exemption from the COVID-19 vaccination requirement.<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">*According to the CDC, individuals are considered fully vaccinated two weeks after their second dose in a two-dose series (such as the Pfizer or Moderna vaccines) or two weeks after a single-dose vaccine (such as Johnson &amp; Johnson\u2019s Janssen vaccine).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Completed attestations will be maintained in a secure system adhering to UCCS\u2019 privacy policies and compliance practices. Information concerning an employee\u2019s completed attestation will not be shared and cannot be requested by supervisors.
