Today, all UCCS students should have received an email from the Wellness Center requiring you to fill out the vaccination attestation form. As part of UCCS\u2019 preparations for the fall semester, all students, faculty and staff will be required to respond to the attestation. The attestation is in line with the University of Colorado\u2019s April 28 decision to require COVID-19 vaccinations for students, faculty, and staff. <\/p>\n\n\n\n
The attestation form requires individuals to attest to one of the following: <\/p>\n\n\n\n
* According to the CDC, individuals are considered fully vaccinated two weeks after their second dose in a two-dose series (such as the Pfizer or Moderna vaccines) or two weeks after a single-dose vaccine (such as Johnson & Johnson\u2019s Janssen vaccine).<\/em> <\/p>\n\n\n\n Completed attestations will be maintained in a secure system adhering to UCCS\u2019 privacy policies and compliance practices. Information concerning a student\u2019s completed attestation will not be shared and cannot be requested by other students, faculty or staff.
