As part of UCCS\u2019 preparations for the fall semester, all students, faculty and staff will be required to respond to the attestation. The attestation is in line with the University of Colorado\u2019s April&nbsp;28&nbsp;decision&nbsp;to require COVID-19 vaccinations for&nbsp;students, faculty, and staff.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The attestation&nbsp;form&nbsp;requires&nbsp;individuals&nbsp;to attest to one of the following:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>I am fully vaccinated*&nbsp;against COVID-19.&nbsp;<\/li><li>I am not&nbsp;yet&nbsp;fully vaccinated against COVID-19; however, I&nbsp;intend&nbsp;to receive a COVID-19 vaccine or the second dose in a two-dose series. If I do not receive the vaccine, I will update this attestation.&nbsp;<\/li><li>I am not fully vaccinated against COVID-19, and&nbsp;I request an exemption&nbsp;from the COVID-19 vaccination requirement.&nbsp;<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><em>* According to the CDC,&nbsp;individuals&nbsp;are considered fully vaccinated two weeks after their second dose in a&nbsp;two-dose series (such as the Pfizer or Moderna vaccines)&nbsp;or&nbsp;two&nbsp;weeks after a single-dose vaccine&nbsp;(such as Johnson &amp; Johnson\u2019s Janssen vaccine).<\/em>&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Completed attestations will be maintained in a secure system adhering to&nbsp;UCCS\u2019 privacy policies&nbsp;and compliance practices. Information concerning a student\u2019s&nbsp;completed attestation&nbsp;will&nbsp;not&nbsp;be shared&nbsp;and&nbsp;cannot&nbsp;be&nbsp;requested by other students, faculty or staff. Data will only be shared in aggregate to determine our progress&nbsp;toward&nbsp;campus herd immunity and\/or to contact students if they have been exposed to known positive cases, not for the purposes of limiting individual student access to courses, programs and services.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You can find frequently asked questions about the attestation process answered&nbsp;at <a href=\"https:\/\/covid19.uccs.edu\/vaccination\">https:\/\/covid19.uccs.edu\/vaccination<\/a>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">While medical and non-medical exemptions are&nbsp;permissible, the COVID-19 vaccine is our most powerful tool to end this pandemic.
